What Is Intramuscular Fat Grafting?
Intramuscular fat grafting is a fat transfer procedure that places purified fat within and around specific muscles to enhance their fullness and shape. Dr. Pazmiño first harvests fat with liposuction, then processes it and injects small amounts in multiple layers through a fine cannula.
Muscle has a rich blood supply, which may help grafted fat survive, although the body always reabsorbs some of the transferred fat. For patient safety, Florida law prohibits injecting fat into or beneath the gluteal muscles, so this procedure is never used in the buttocks; buttock enhancement is performed through the ultraBBL®, which places fat only beneath the skin.
Dr. Pazmiño’s research showing that BBL deaths were concentrated in South Florida helped build the case for HB 1471, Florida’s 2023 law requiring ultrasound guidance during gluteal fat grafting. As a director of the Florida Society of Plastic Surgeons, he helped recommend elements of the bill, and he testified before the Florida Board of Medicine on the safety rules that preceded it. The Florida House of Representatives cites his study in its analysis of the 2024 law that followed.
