Breast Augmentation Incisions

Breast Augmentation Incisions

Dr. Pazmiño may place incisions in several locations. The choice depends on your body type, lifestyle, and personal preference.
This incision is very popular with patients because it is difficult to detect at the border of the darker skin around the nipple and the lighter skin of the breast. This is also an excellent option for patients whose breasts may be uneven or may require a lift.
The inframammary incision is placed within the natural crease beneath the breast, allowing the resulting scar to remain well hidden. This approach offers excellent visibility during surgery, giving Dr. Pazmiño precise control over implant placement and positioning. It is one of the most commonly used incision techniques and is suitable for most implant types and sizes.
The transaxillary incision is made within a natural crease of the armpit, leaving no scar on the breast itself. Using specialized techniques, Dr. Pazmiño creates a pocket for the implant through this discreet incision while maintaining precise implant placement. This approach is an excellent option for patients who prefer to avoid scars on the breast, although it may not be suitable for every anatomy or implant type.
This incision, also known as the “lollipop” incision, begins around the nipple and continues down the center of the breast. This is excellent for patients who would like an implant but also need significant lifting of the breast at the same time. Through a single incision, a patient can combine two surgeries in one (breast lift AND a breast augmentation)!

Implant Placement

During the surgery, Dr. Pazmiño can place the implants either under the breast tissue (subglandular position) or under the chest muscle (subpectoral position).
Implants placed beneath the chest muscle offer a few advantages over those placed only beneath the breast tissue. These advantages include a reduced risk of capsular contracture (post-operative tightening around the implant) and less interference with mammogram examinations. A possible disadvantage includes soreness in the first few days after surgery.
Dr. Pazmiño will answer all of your questions so that you can decide which position is right for you.

Your Implant Options

Breast Augmentation Choices: Which Incision Is Right For You?

Women choosing breast augmentation can select from a number of different implant profiles or heights. During your consultation, measurements of your breasts and chest will be taken, and you will learn the advantages and disadvantages of each profile. You will then be able to decide which profile will meet your aesthetic goals!
Breast implants are silicone shells filled with either saline (saltwater) or silicone gel. Both implant types are safe, and each offers its own advantages. The decision to use saline or silicone gel breast implants is based on your aesthetic goals, your body shape, and your personal preferences.
The U.S. Food and Drug Administration (FDA) has approved both implant options for breast augmentation surgery. Dr. Pazmiño usually offers silicone gel-filled breast implants, which are much softer and feel more natural than saline-filled implants.
Breast implants can come in round or pear-shaped (anatomic) shapes. Advantages of the round shape include more fullness in the upper part of the breast (more cleavage). At your consultation, you’ll see examples of each and be able to try them! This will allow you to choose the one that best fits your goals and body type!
Breast implants have either a smooth or textured surface. This becomes particularly important for those patients who choose a pear-shaped implant or who have had capsular contractures from previous implant surgery. During your consultation, you will feel the difference and choose the most suitable option.
At Miami Aesthetic, we have every breast implant size and shape! During your consultation, you will learn how the size of the implant affects your ultimate result. By using digital imaging and trying on the implants yourself, you can select one that complements your figure and meets your aesthetic goals.

Preparing for Breast Augmentation

Preparing for your breast augmentation can help ensure a smoother procedure and recovery. During your consultation, Dr. Pazmiño will review your medical history, discuss your goals, and provide personalized pre-operative instructions. In general, patients should:
  • Stop smoking and using nicotine products as directed before surgery.
  • Avoid blood-thinning medications and supplements if approved by your physician.
  • Arrange for someone to drive you home and assist you during the first 24 hours.
  • Fill prescriptions before your procedure.
  • Prepare a comfortable recovery area with pillows, medications, water, and easy-to-reach essentials.
  • Follow all fasting instructions before surgery if general anesthesia will be used.

Careful preparation helps promote a safe procedure and a more comfortable recovery.

How Is Breast Augmentation Performed?

How Is Breast Augmentation Performed?

A breast augmentation at Miami Aesthetic takes approximately 45–90 minutes and is performed as an outpatient procedure. Here is a more detailed overview of the surgery:
To ensure patient comfort and safety, breast augmentation begins with general anesthesia. Our surgical team monitors vital signs throughout the procedure while the patient remains asleep and pain-free. Anesthesia is essential for creating an optimal environment for a smooth surgical process.
Dr. Pazmiño makes a carefully planned incision based on the patient’s anatomy and aesthetic goals. Standard options include:
  • periareolar (around the areola)
  • inframammary (in the breast fold)
  • transaxillary (through the underarm).
Each approach is designed to minimize visible scarring while allowing precise access for implant placement.
Once the incision is made, a pocket is created to accommodate the breast implant. This pocket may be positioned either above or below the chest muscle, depending on factors such as body type and the desired result. Placing the implant beneath the muscle can improve contour and reduce the risk of certain complications, offering a more natural transition from chest wall to breast.
After the implants are positioned and balanced, Dr. Pazmiño closes the incisions in layers. Internal sutures help support healing tissues, while the outer skin is sealed with fine stitches or surgical tape. This layered closure is critical for promoting smooth healing and minimizing scar visibility.
Before the procedure concludes, our surgical team performs a thorough evaluation to ensure the implants are properly seated and the results meet the planned expectations. The chest is then cleaned, bandaged, and dressed for the initial recovery period. Every detail is carefully managed to support both safety and aesthetic outcome.

Recovery After a Breast Augmentation

Recovery After a Breast Augmentation

Patients who undergo a breast augmentation in the morning are home by lunchtime and can return to work in 1 to 3 days. Each patient will receive an appropriate post-op bra or garment to help the implants heal in the correct position. Most swelling from the surgery will subside in 2-4 weeks, while implants may take 4-8 weeks to achieve their final position. Incision lines will continue to fade for months or years. Here is a more detailed overview of the procedure:
While every patient’s healing timeline is unique, following your post-operative instructions can help minimize discomfort, reduce the risk of complications, and support beautiful, long-lasting results.
  • Wear your surgical bra as directed. Your post-operative bra helps support your breasts, minimize swelling, and encourage your implants to settle into their proper position. Wear it exactly as instructed by Dr. Pazmiño.
  • Get plenty of rest. Take it easy during the first few days after surgery. Light walking is encouraged to promote healthy circulation, but avoid strenuous activity, heavy lifting, and upper-body exercise until you receive clearance.
  • Sleep on your back. For the first several weeks, sleep on your back with your upper body slightly elevated. This helps reduce swelling, protects your incisions, and prevents unnecessary pressure on your healing breasts.
  • Take medications as prescribed. Use your prescribed pain medication, antibiotics, and any other medications exactly as directed to help manage discomfort and reduce the risk of infection.
  • Stay hydrated and eat nutritious foods. Drinking plenty of water and maintaining a balanced diet rich in protein, fruits, and vegetables can support your body’s natural healing process.
  • Care for your incisions. Keep your incision sites clean and dry, and follow all wound care instructions provided by our team. Avoid applying creams or ointments unless specifically recommended.
  • Avoid smoking and nicotine. Smoking, vaping, and all nicotine products can significantly impair healing by reducing blood flow to the surgical area. Patients should avoid nicotine before and after surgery as directed by Dr. Pazmiño.
  • Be patient with your results. It’s normal for your breasts to appear swollen, firm, or sit higher on the chest immediately after surgery. As healing progresses over the following weeks and months, swelling will continue to improve, implants will settle into their final position, and your results will become more natural.
  • Attend all follow-up appointments. Regular post-operative visits allow Dr. Pazmiño to monitor your healing, answer questions, and ensure your recovery is progressing as expected. If you experience severe pain, excessive swelling, fever, or any unusual symptoms, contact our office promptly.
Proper sleeping position is an important part of recovery after breast augmentation. During the first several weeks, patients should sleep on their back with the upper body slightly elevated using pillows or a recliner. This helps reduce swelling, protects the incisions, and prevents unnecessary pressure on the healing breasts.
Avoid sleeping on your stomach or side until Dr. Pazmiño confirms it is safe to do so, as these positions may place stress on the implants and healing tissues. Most patients can gradually return to their preferred sleeping position after several weeks, depending on their healing progress.

How Long Do Breast Implants Last?

Breast implants are durable and designed to last for many years, but they are not considered lifetime devices. Many patients enjoy their results for 10 to 20 years or longer without needing implant replacement.
Implants do not need to be replaced on a set schedule. Instead, replacement is only recommended if complications develop, such as implant rupture, capsular contracture, changes in breast appearance, or if your aesthetic goals change over time.
Routine follow-up appointments and, for silicone implants, periodic imaging as recommended by the FDA can help monitor implant integrity and maintain your long-term breast health.

Scar Care Tips

Most breast augmentation scars fade significantly with time and proper care. To support optimal healing:
Keep incisions clean and dry.
Use silicone gel or silicone sheets if recommended by Dr. Pazmiño.
Protect scars from sun exposure with SPF 30+ once healed.
Avoid smoking and nicotine, which can impair healing.
Attend follow-up visits to monitor your healing.
Scars typically appear pink or firm at first and gradually soften and fade over the following months.

Risks Of Breast Augmentation

At Miami Aesthetic, our first priority is your safety. Every patient is carefully examined, and appropriate screening tests are performed before your surgery. Operative precautions are taken before, during, and after your surgery. The recovery of each patient is carefully followed by Dr. Pazmiño and his staff. This helps our patients recover comfortably and consistently get the results they want.
Although rare, post-operative complications from surgery are possible. They may include infection, capsular contracture (post-operative tightening around the implant), a change in nipple sensation, and poor healing, which may require further surgery.
A rupture, or leakage, of the implant as a result of injury or the normal compression and movement of your breast is uncommon. If a saline implant ruptures, it will simply deflate as your body safely absorbs the saltwater. If a silicone implant leaks, you will need to see Dr. Pazmiño. Regular monitoring of breast implants is recommended to ensure continuing breast and implant health.

Frequently Asked Questions (FAQs)

Breast implant surgery allows for greater control over breast shape, size, and projection, which may not be achievable with fat transfer alone. Implants can also be placed under the chest muscle to create a more defined and lasting contour, especially for patients seeking significant enhancement.
Start by verifying that your surgeon is certified by the American Board of Plastic Surgery or another reputable organization, ensuring rigorous training. A strong understanding of the human body, the skill to make inconspicuous incisions, and a clear record of successful outcomes are all key indicators of expertise.
Breast augmentation is typically considered a cosmetic surgical procedure; however, in some instances—such as after mastectomy or trauma—it can be part of reconstructive surgery. It helps restore breast volume and shape, especially when addressing congenital asymmetries or post-surgical changes.
Silicone implants feel more natural due to the cohesive fill material used inside. While they may require a slightly larger incision for placement compared to saline implants, many patients prefer the look and feel of silicone for long-term satisfaction.
This combination is often part of a mommy makeover, a comprehensive approach to restoring the body after pregnancy. It addresses changes in the breasts and midsection while reducing excess fat in areas like the outer thighs, creating a more balanced and youthful appearance.
While both smooth and textured shells are available, there are specific risks involved with textured implants that have emerged in recent years. Very rare cases of a condition known as BIA-ALCL have been linked to textured implants. Your surgeon will help you weigh the benefits and risks to make the safest choice for your needs.
